What form data is collected
When you submit the contact form, Claims Attachment Gateway may collect your first name, last name, work email, company, role/title, organization type, selected interests, message, page source, and technical anti-spam signals. When you submit the gated resource form, we collect your name, business email, organization, opt-in consent, page source, and technical anti-spam signals.
How form data is used
Form data is used to respond to B2B sales inquiries about fax intake, fax number migration, outbound fax fallback, vendor partnerships, partner project engagements, and healthcare organization workflows; to send the requested resource; and to follow up about how the PHI eXchange might help with related fax planning.
Your control
You can request deletion of your information at any time by emailing [email protected]. We do not sell your information. We do not share it with third parties for marketing.
No PHI submission
Do not submit PHI, patient information, claim documents, medical records, or confidential health information through any form on this website. The forms are not designed to receive patient, claim, or medical documentation.
Cookies and analytics
This site uses first-party session cookies for CSRF protection and rate limiting on forms, and may use analytics or marketing tracking tools (Google Analytics, HubSpot, LinkedIn Insight) added during deployment. Where required by applicable law, we will display a consent notice before non-essential tracking is loaded.
Bot protection (Cloudflare Turnstile)
Forms on this site are protected against automated abuse by Cloudflare Turnstile. When you submit a form, your browser exchanges a small challenge with Cloudflare and sends a verification token to us. We send that token, along with your IP address, to Cloudflare to confirm the challenge was completed by a human. Cloudflare may set first-party cookies on the challenge subdomain. How we send and receive email
Form submissions are delivered to our team via authenticated SMTP through Hostinger’s mail infrastructure. We do not store the contents of your form submission on this site beyond what is required to send the message and respond to you.
